Are you a carer? If you look after relatives, friends or neighbours who cannot manage alone due to illness, old age, disability etc., then you are a carer - and we would like to help you. We are interested in identifying carers, especially those people who may be caring without help or support.  We know that carers are often “hidden” looking after a family member or helping a friend or neighbour with day to day tasks and may not see themselves as a carer.

We feel that caring for someone is an important and valuable role in the community, which is often a 24 hour job that can be very demanding and isolating for the carer.  We further believe carers should receive appropriate support by way of access to accurate information on a range of topics such as entitlement to benefits and respite care and, not least, a listening ear when things get too much.

As a carer you are also entitled to have your needs assessed by Carers Wellbeing Service (via the Carers Trust, Heart of England).  A carer’s assessment is a chance to talk about your needs as a carer and the possible ways help could be given.  It also looks at the needs of the person you care for.  This could be done separately, or together, depending on the situation.  There is no charge for an assessment for further details contact 024 7610 1040.

If you are a carer this is an opportunity to let the surgery know so that we can update your records and provide you with the information pack we have put together for carers.

We have a 'Carers Corner Display Shelf' in Reception with information including useful information on contacts and support services, how you can register with us to get a Carers Pack and a suggestion box to help improve this service. Our Carers Pack contains lots of useful information and includes a free 'message in a bottle' which is a great way to hold your emergency medical and contact details. We send out regular updates to the pack too in the form of a 6-weekly newsletter.

To register as a carer and get you 'Carers Pack' fill in the form below and either hand it into Reception, post it or email it to brooksidereception@nhs.net
