New Patient Registration
Registration with the practice is open to anyone living within the practice area that is not already registered with a GP. If you live within the practice area and are currently registered with another GP then registration is still possible but we would ask that you speak to our practice manager in the first instance.
To register please collect a set of forms from reception for each person registering. The receptionist will explain how the practice works and give you a Practice Information Leaflet.
Patients are now registered with the Practice not with an individual GP. However, you will be given the opportunity to choose a preferred GP. You may choose to see only your preferred GP, but there may be times when you will be asked to see an alternative doctor (for example because the service requested is not available from your chosen GP).
We offer all new patients from 14 upwards an appointment for a new patient check with a practice nurse. This is best arranged at the time of registering.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice. If your first visit to the Practice is for an appointment please attend 15 minutes before your appointment to allow our Reception staff time to enter your details onto our system. You will also need to complete a New Patient Questionnaire (which can also be printed off, see below) and bring that with you.
